Use this W-2 form to report wage and withholding information. This 6-part carbon-interleaved form includes employer copies A (federal, red scannable), 1 (state) and D (file) and employees copies B (federal), 2 (state) and C (file) which are pre-inserted into an envelope. When you use your typewriter or pin-fed printer, the mailing address will also be printed on the envelope. You just seal and mail! The employer and employee forms are side-by-side, reducing the form thickness, so you can print legibly on all parts at high speeds. Forms are 8.5" wide (for a total width of 17")due to format change in 2001. Mail employer copies in a batch to the SSA. Employee copies are ready to mail.
